Pronation Problems

I have been an overpronator since I was 12 (right foot). I'm 46 now, and the problems just keep getting worse. I had sclerosing injections done for a neuroma
which did a pretty good job. I also wear custom orthotics which don't solve everything but prevent me from being in complete agony. However, my ability to
ambulate is getting pretty impaired. It is difficult to "toe off" on the right foot and by the end of the day I'm in a fair anount of pain...this, by remaining as sedentary as possible. Walking around to do errands
causes problems. I feel like I'm doing less and less, becoming a cripple at 46. I know that there is a limit to what a podiatrist can do, but what are my options? Should I be hobbling like this? I always wear
sneakers specifically designed for overpronators, btw,
with the orthotics, and my foot still cannot be coaxed
into a normal gait pattern, it seems.
